To sign up for your CSA share, please fill out the online form below, and mail your payment to the farm.

Payment options:

1. Pay entire season upfront: Mail in one check for the full amount of your share.

2. Pay in two installments: Mail two checks at once, each for half of the cost of your share (one with today's date, the other dated 7/15/17).

Note: Your CSA share is not reserved until payment is received. Once we receive your form and check(s), we will send you an email confirmation and receipt.

Please mail payment for the cost of the share selected above. You may pay the entire cost upfront with one check, or split the payment across two installments (mail both checks at once, and date one with today's date and the second 7/15/17). CSA membership forms and checks are due by May 30, 2017. We have a limited number of shares available, so we recommend sending in your check to reserve your membership as soon as possible. HEALTH CARE REIMBURSEMENT: Our CSA shares are eligible for a health care reimbursement from some providers. See our website for more information on available options. PARTNER SHARES: The FairShare CSA Coalition provides financial assistance though the Partner Shares program. Please contact FairShare for more information on the application process. If you would like to donate to the Partner Shares program, check the box below and specify the amount. You can add this to the total amount submitted for your share, and we will send the full donation amount to FairShare.
As a member of Plowshares & Prairie Farm, LLC, I understand I will be receiving a box of produce each week from the week of June 5th, 2017 through the week of October 16th, 2017. It is my responsibility to pick up my box from the drop-off location each week between the designated hours. If I cannot pick it up, I will find someone else to pick it up and give them instructions for where and when to get it, or my box will be donated. I understand that my farmers will be working to provide me with fresh, healthy produce, but that mother nature will inevitably introduce some variability into the season.
